Persistan ve Rekürren Primer Hiperparatiroidizm: İntraoperatif Tamamlayıcı Yöntemler, Cerrahinin Temel Prensipleri, Diğer Tedavi Seçenekleri

Reoperatif paratiroid cerrahisi deneyimli cerrahlar için bile zordur. Kür oranları primer cerrahiye göre daha düşüktür. İyi anatomik ve embriyolojik bilgi önemlidir. Ameliyat öncesi kapsamlı bir cerrahi strateji planlanmalıdır. Olası komplikasyonlardan kaçınmak için odaklanmış paratiroid cerrahisi yapılabilmesi amacıyla gözden kaçan bezi bulmak için ameliyat öncesi görüntüleme yöntemleri kullanılmalıdır. Önemli gelişmelerden birisi gözden kaçan paratiroid bezlerinin bulunmasına yönelik yeni yardımcı yöntemlerdir. Cerrahi başarı oranını artırmak için ortotopik ve olası ektopik yerleşimler cerrah tarafından iyi bilinmelidir. Reoperatif paratiroid cerrahisi, primer paratiroid cerrahisine kıyasla farklı bir yaklaşım gerektirir. Tiroid bölgesine giriş için kesi ve yolun seçimi, yardımcı yöntemlerin kullanımı ve intraoperatif sinir monitörizasyonu temel prensiplerdir ve ayrıca titiz bir diseksiyon gerektirir. Cerrahi kür elde etmek zordur ve yüksek cerrahi dikkat gerektirir. Postoperatif komplikasyon oranları primer paratiroid cerrahisine göre daha yüksektir. Ameliyat olamayan bir hastada diğer tedavi yöntemleri ve medikal tedavi seçenekleri değerlendirilebilir. (SETB-2023-06-100)

Reoperative parathyroid surgery is challenging even for experienced surgeons. Cure rates are lower than primary surgery. Good anatomical and embryological knowledge is important. Preoperatively, a comprehensive surgical strategy should be planned. Pre-operative imaging modalities should be used extensively to find the overlooked gland to have a possibility to perform focused parathyroid surgery to avoid possible complications. One of the important developments is the new ancillary methods to find overlooked parathyroid glands. Orthotopic and possible ectopic locations should be known well by the surgeon to increase the surgical success rate. Reoperative parathyroid surgery needs a distinctive approach compared to primary parathyroid surgery. Basic principles include the selection of the incision and route for entering the thyroid region, use of ancillary methods, and intraoperative nerve monitoring and also require a meticulous dissection. Obtaining a surgical cure is difficult and high surgical caution is needed. Post-operative complication rates are higher compared to primary parathyroid surgery. Other treatment methods and
medical treatment options may be evaluated in a patient who cannot undergo surgery.
