| 3. | The prevalence of skin disease in the geriatric population Ilteriş Oğuz Topal, Gonca Gökdemir, Adem Köşlü Pages 28 - 32 Background and purpose: The geriatric population is composed of the people over 65 years of age. Skin disease is common in this population because of wide variety of dermatological conditions due to degenerative and metabolic changes occurring throughout the skin layers. Studies about prevalence of skin disease in the elderly people have been reported increasing recently. The aim of this study is to establish the prevalance of skin diseases in geriatric population in our outpatient clinic. Methods: A total of 1108 geriatric patients who were older than 65 years-old was included in this study between October 2006 and May 2007. Each patient was examined by same physician, and the dermatological signs and demographic features were recorded. Results: Total of 1108 patients, 65-98 years old (mean 72,4) were evaluated. There were 447 women (40,4%) and 661 men (59,6%). The distribution of patients according to dermatological problems were as follows; xerosis 49,6% (550), fungal infections 47,6% (528), benign neoplasms 39,2% (435), solar lentigo 24,1% (268), eczema 12,9% (144), pruritus 12,3% (137), premalignant lesions 7,9% (88), malignant neoplasms 5,6% (63), viral infections 4,5% (50), bacterial infections 2,4% (27), acne rosacea 1,6% (18) and the others 3,1% (35). In our clinic, the most common diseases were benign neoplasms (% 41,6), xerosis ( % 36,9), and fungal infections (% 33,5). Conclusions: Our results were similar to the literature. It is needed to perform further studies to point out of importance of having preventive measures because of the higher risk of many dermatological diseases among this group. |

| 4. | Mixed tubulopapillary hidroadenoma and syringocystadenoma papilliferum of the thigh occuring on lentiginous hyperplasia Ayşin Karasoy Yeşilada, Mahmut Ulvi Kayalı, Özay Özkaya, Damlanur Sakız, Lütfü Baş Pages 33 - 34 Syringocystadenoma papilliferum is an uncommon cutaneous adnexal benign tumor most commonly located on the scalp or face. It frequently arises from a nevus sebaceus. This lesion rarely arises on the trunk and extremities. Mixed tubulopapillary hidroadenoma is a recently proposed category for adnexal tumors that remains controversial. It encompasses a spectrum of lesions from papillary ecchne adenoma (PEA) to tubular apocrine adenoma (TAA). Eight cases of tubular apocrine adenoma or papillary eccrine adenoma with syringocystadenoma papilliferum have been reported. We present a case of 56-year old female patient with tubulopapillary hidroadenoma and syringocystadenoma papilliferum with lentiginous melanocytic hyperplasia on the thigh. |

| 5. | Comparison of phacoemulsification effect on intraocular pressure in patients with pseudoexfoliative and senile cataract A. Burcu Dirim, Ersin Oba, Esra Türkseven, Ceylen Uslu, Hüseyin Fındık, Ulviye Yiğit Pages 37 - 40 Aim: ‘To determine intraocular pressure response to phaco-emulsification in patients with pseudoexfolative and senile cataract. Methods: We studied prospectively 40 eyes of 40 patients of which 20 of them with exfoliation (1.group) and 20 without exfoliation senile cataract (2.group) in the Ophthalmology Clinic of Sisli Etfal Teaching Hospital between the dates January 2005-June 2006. In this study we included over 50 ages, wide iridocorneal angle and cataract patients. In all patients intra-ocular pressure (IOP) measurement of preope-rative and postoperative 1.day, I.month, 3.month, best cor¬rected visual acuity and routine follow up were performed. Results: in the first and second groups, the mean preoperative IOPs were 21,20±6.30 mmHg and 18.46±6.85. There was no significant difference between two groups (p>0,05). Postoperative 3.month IOP measurements were I6.5±2.7 mmHg in the first group and 15.54±2.57 mmHg in the second group. Conclusions: Postoperative mean IOP values decreased significantly in both pseudoexfoliative and senile without exfoliation cataract groups (p<0.001 and p<0.05). In this study there was no statistically significant difference between preoperative and postoperative mean IOP reduction values in both groups (p>0.05). |
| 6. | Comparison of the effects of oral omeprazole, famotidine and ranitidine on gastric volume and ph in elective surgery patients Hale Dobrucalı, Leyla T. Kılınç, Ulufer Sivrikaya, Melahat K. Erol, Ayşe Hancı, Metin Bektaş Pages 41 - 46 Objective: In our study we aimed to compare the effects of H2 reseptor blockers {famotidin and ranitidine) and H-K ATPase inhibitor (omeprazole) on intragastric volume and pH in patients undergoing elective surgery under general anesthesia. Study design: After the approval by the Medical Ethics Committee of our hospital, 80 patients in ASA I-II physical status randomized into four groups. The patients were given 20 mg omeprazole (group I) or 150 mg ranitidine (group 2) or 40 mg famotidine (group 3) by orally with 10 ml water 2 hours before anaesthesia induction. The patients in control group (group 4) were given 10 ml water alone. pH electrode was in¬serted transnasally and than gastric volume and pH measured at peroperative and 30 minutes postoperative période. Results: Gastric volume was significantly higher in group 4 than other three groups at anaesthesia induction and extubation (p<0.001). Gastric volume values were significantly lower in Group 2 than group 1 and 3 (p<0.001). Number of high aspiration risk patiepts were significantly lower at group 1, 2, 3 than group 4 (p<0.05). Gastric volume pH values were lower at group 4 than the other groups peroperatively and recovery period (p<0.001 ). Conclusion: This study showed omeprazole, famotidine and ranitidine can be used in to prevent gastric acit aspiration during general anesthesia safely. |

| 7. | Giant schwannoma cases of neck Seyhan Alkan, Burhan Dadaş Pages 47 - 49 Schwannomas (neurilemmomas) are solitary neurogenic tumors which are observed rarely and originate from peripheric, cranial or autonomic nerve cells. Twenty-five to 45 percent of extracramal schwannomas occur in the head and neck region. They grow slowly and frequently manifest themselves as asymptomatic masses in the neck. Imaging methods such as ultrasonography (USG), computed tomography (CT), magnetic resonance imaging (MRI) and angiography are helpful in the diagnosis. A 13-year-old boy presented with a slow- growing, painless, 15x10x10 cm mass on the left side of his neck that had been noted 2 years previously. During surgery, a well-encapsulated mass originated from the cervical sympathetic chain was observed. Histopathologic examination was reported as a schwannoma. |
| 8. | A case report of a placenta previa in a Jehova‘s witness patient Melahat Dönmez, Alev Atış, Günseli Özdemir, Savaş Özdemir, Özlem Oruç, Selahattin Mısıroğlu Pages 50 - 51 Introduction: The most frequent causes of perinatal and maternal mortality and morbidity is antepartum bleeding. Placenta previa is very important in antepartum bleeding. The incidence is 03-03 %. It can be total, partial, marginal or low segment placed placenta previa. Bleeding in placenta previa cases is an unpainful hemorrhage which is not seen until the 2.trimester.Death caused by placenta previa decreases with adequate blood transfusion caeserian/sectio which advised by Bill in 1927Jehovah’s witnesses is a religion group which has a lot of member in the world who don’t accept the transfusion of blood and blood products because of their belief thus there is a difficulty in this patient’s treatment. In this case, we discussed a placenta previa case belongs to Jehovah’s witnesses. Case: 36 years old, according to the last menstrual bleeding 31-32 weeks pregnant women came to our clinic with vaginal bleeding complaint. MGS04 treatment was started as tocolysis, steroid was given for lung maturation, and followed for bleeding. After 3 weeks the Hgb level ped to 10,6gr/dl so iv Fe treatment was started. After 1 week, because of bleeding, patient was undergone to cesearianlsection. No complication was seen per or postoperatively. Baby was followed in newborn sendee for respiratuvar problems by CPAP. Results: Hemostasis and surgical procedures (radical operations if needed) must be applied promptly in cases which transfusion can’t be performed. Also ivferrum therapy was prepared for probability of bleeding. In these patients iv ferrum therapy can be more effective over eritropoetin; because of the shorter affecting time. But in patients with severe anemia increase risk of post operative mortality and morbidity must be remembered. |
