Çocukluk çağında görülen sindirim sistemi perforasyonları

Amaç: Çocukluk çağında sindirim sistemi perforasyonu gö¬rülen olguların literatür eşliğinde incelenmesi.
Gereç ve Yöntem: 1994-2002 yılları arasında sindirim siste¬mi perforasyonu tanısı alan 35 hasta geriye dönük olarak değerlendirildi. Olgular: yenidoğan, infant ve çocuk olmak üze¬re üç gruba ayrıklı.
Bulgular: Yenidoğan grubunda, sindirim sistemi perforasyonunun en sık nekrotizan enterokolit (n=10) nedeniyle, ileum- da (n=9) geliştiği gözlendi. Bu hastalara genellikle ileostomi (n=8) yapıldı ve mortalité oraıv %35.2 oldu. Infant grubunda, bir mide ve bir kolon perforasyonu gözlendi. Bir hastada da perforasyonun yeri bulunamadı. Bu grupta sadece bir hasta yaşadı. Çocuk grubunda ise en sık perforasyon sebebi karın travması (n=4) ve koroziv madde alimiydi (n=4). Bu olgulara genellikle primer tamir (n=6) yapıldı ve mortalité oranı %26.6 oldu.
Sonuçlar: Sağlık teknolojisindeki gelişmeler çocuklarda görülen sindirim sistemi perforasyonlarmın tanı, tedavi ve takip yöntemlerine ciddi katkılar sağlamıştır. Ancak yaşamı tehdit eden bu sorun günümüzde bile ciddiyetini korumaktadır. Erken tanı ve uygun cerrahi tedavi mortalité oranını düşüren en önemli faktörlerdir.

Results: In newborn group, gastrointestinal perforation frequently observed in ileum (n=9) due to necrotizing t colitis (n=10). Ileostomy (n=8) were performed and me rate was 35.2% in this group. In infant group, one st and one colon perforation were observed and the perfc zone could not be detected in one patient. Only one / survived in this group. In children group, blunt abdomir uma (n=4) and corrosive intake (n=4) were the most ce causes of perforation. Primary suture (n=6) were per] for this groups’ cases and mortality rate was 26,6%.
Conclusion: The advances of medical technologies up\ the diagnosis, treatment, and follow-up of childhood ga testinal perforations. However, today, gastrointestinal rations still remains to be a life-threatening condit childhood. Early diagnosis and appropriate surgical vention are the most important factors for decreasing, lity rates of this problem.
