Fekal impaksiyona bağlı spontan kolon perforasyonu

Konstipasyon ve fekal impaksiyon özellikle yaşlı hasta grubunda sıkça görülebilmektedir. Fekal impaksiyona bağlı kolon iskemisi ve perforasyon ise, oldukça nadir görülen bir durumdur. Komplike olmamış fekal impaksiyon vakalarında cerrahi müdahaleye nadiren gerek duyulur ve bu durum literatürde de az sayıda bildirilmiştir. Bu olgumuzda Genel Cerrahi polikliniğine karın ağrısı ve karında şişkinlik şikayetleriyle başvuran yaklaşık on gündür defekasyonu olmayan, ilk başvurusunda elle halas uygulanarak ertesi gün taburcu edildikten üç gün sonra ani başlayan karın ağrısı ve karında şişkinlik şikayetleriyle Acil Servise başvuran ve çekilen karın tomografisi sonucunda sigmoid kolonda fekalom ve perforasyon saptanarak sol hemikolektomi ve Hartmann prosedürüyle kolostomi uygulanan hasta sunulmuş ve güncel literatür eşliğinde tartışılmıştır.

Spontaneous colon perforation due to fecal impaction

Constipation and fecal impaction, can be seen frequently especially in elderly patients. On the other hand, colonic ischemia and perforation due to fecal impaction is a rare condition. Uncomplicated cases of fecal impaction rarely requires surgical intervention, and a few in this situation have been reported in the literature. In this case, a patient who admitted to general surgery clinic for about ten days without defecation, complaints of abdominal pain and abdominal bloating; in his first application manual extraction of petrified feces was performed and next day discharged from the hospital; three days after applied to the Emergency Service with the sudden onset of abdominal pain and abdominal bloating complaints; and after abdominal computed tomography imagining, fecaloma and perforation in the sigmoid colon identified; and colostomy with Hartmann procedure was performed after left hemicolectomy; is presented and discussed with the current literature.
